Copyright Infringement Notice
To submit a copyright infringement notice, please provide a written communication that includes the following information:

Misrepresentations
Please be aware that under 17 U.S.C. § 512(f), any person who knowingly materially misrepresents that material or activity is infringing may be liable for damages, including costs and attorneys’ fees.
Counter-Notification
If you believe that material you posted or submitted to Formed From Light was removed or disabled as a result of mistake or misidentification, you may submit a written counter-notification.
To be effective, a counter-notification must include the following information:
- Your physical or electronic signature.
- Identification of the material that was removed or disabled and the location where the material appeared before it was removed or disabled.

- A statement that you will accept service of process from the person who submitted the original DMCA notice or an agent of that person.

Repeat Infringer Policy
