Copyright Infringement Notice

To submit a copyright infringement notice, please provide a written communication that includes the following information:

Misrepresentations

Please be aware that under 17 U.S.C. § 512(f), any person who knowingly materially misrepresents that material or activity is infringing may be liable for damages, including costs and attorneys’ fees.

Counter-Notification

If you believe that material you posted or submitted to Formed From Light was removed or disabled as a result of mistake or misidentification, you may submit a written counter-notification.

To be effective, a counter-notification must include the following information:

  2. Identification of the material that was removed or disabled and the location where the material appeared before it was removed or disabled.
  3. A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled because of mistake or misidentification.

  5. A statement that you consent to the jurisdiction of the federal district court for the judicial district in which your address is located, or, if you are located outside of the United States, that you consent to the jurisdiction of any judicial district in which Formed From Light may be found.
  6. A statement that you will accept service of process from the person who submitted the original DMCA notice or an agent of that person.

Repeat Infringer Policy
